  • Annual periodic exam – 100% covered
  • Annual set of Bitewing X-rays – 100% covered
  • Two cleanings per year (non-periodontal) – 100% covered
  • Panoramic X-ray every 5 years – 100% covered
  • Fillings – 15% discount
  • Oral surgery (Excluding Implants) – 15% discount
  • Root canals – 15% discount
  • Periodontal cleanings – 15% discount
  • Crowns (excluding veneers) – 15% discount
  • Dentures and partials – 15% discount


* * Whitening, veneers, dental implants, and clear aligners are not included.


Plan Pricing

  • Individual: $325/year
  • Dual*: $600/year
  • Family (3)** : $800/year
  • Family (4)** : $1000/year
  • Each additional: $150


*The dual plan is for spouses or parent/child only



**The family plan includes family members and children who are under 18 or under 23 if enrolled in college full time


Important Plan Details

  • This plan is a discount plan, not dental insurance
  • No refunds will be issued at any time if you decide not to utilize the plan
  • Benefits begin immediately and last 12 months from purchase
  • The plan cannot be used in conjunction with another plan
  • Plan cannot be combined with insurance or other promotions
  • Payments are due on the date of service, in accordance with our office financial policy
  • The plan cannot be used in conjunction with any third-party financing such as Care Credit
  • The plan cannot be used for referrals to specialists
  • All services must be performed at Spring Hill Dental Center
